You can contact a doctor, nurse or other healthcare professional online using a website called Klinik.

Urgent care requiring on the day assessment

To request an urgent appointment for today (Monday to Friday) during opening times:

  • contact us online
  • If you or a relative/associate cannot access the internet to complete a KLINIK form, please call reception on 01208 812 222
  • Please note: Calling the surgery for an appointment will not mean you receive an appointment quicker than using KLINIK

When you get in touch, we’ll ask what you need help with.

Any urgent care will be under the care of an acute clinician, and may be dealt with over the phone, or at the surgery.

We may not be able to offer your choice of clinician.

Routine appointments

To request a routine appointment please complete a KLINK form. If you or a relative cannot access the internet please call reception on 01208 812 222

When you get in touch, we’ll ask what you need help with.

We will use your answers to choose the most suitable doctor, nurse or healthcare professional to help you.

Using Klinik is the quickest and most efficient way for you to tell us about a medical problem. You will not require a login or password to access Klinik, however, if you already use the NHS app and login using those details the form will be pre-populated with your name, date of birth and address.

If you have a new medical problem

Klinik will take you through a series of questions. If you are unable to access or use Klinik online, the reception team will ask you the same questions and will input your answers on your behalf. Depending on the nature of your main symptom you will be asked some further questions.

This will help us direct your request to the right person and ensure urgent problems are highlighted quickly. We will have a duty team including an experienced GP reviewing new submissions throughout the day and acting on them as appropriate.

This may mean they telephone you for more information, arrange a telephone consultation, send you a text, book a face-to-face consultation or schedule a home visit.

If you want to request a follow-up for a previous medical problem

Use the “existing health problem” option if your symptoms have not improved. You will be asked to fill in some details and you may want to include information about why you need a follow-up, who your last appointment was with and how long ago it was.

Out of hours

To contact a doctor, please phone 01208 812 222. Your call will be diverted to a service providing cover for the emergencies.

The body responsible for providing out of hours service is NHS Kernow.

Between the hours of 6pm to 8:30am Monday to Friday and at weekends, there will be a doctor available for emergencies only.

Nurse appointments and blood tests

If you wish to make an appointment with the nurse or for a blood test please call us on 01208 812 222 and select option 2 for reception.

Will I be able to see a GP?

One of our central aims is to prioritise continuity of care. If you have a long-term problem or need routine follow-up with a specific clinician, your query will be reviewed by the team and actioned according to clinical need.

Another strength of this new approach is that we have oversight of all daily requests for appointments, and this will help to ensure that your appointment is with an appropriately qualified health professional.

We are lucky enough to have a broad multi-disciplinary team including practice nurses, health care assistants, advanced nurse practitioners, emergency care practitioners and musculoskeletal practitioners.

However you choose to contact us, we may offer you a consultation:

  • by phone
  • on a video call
  • by text or email

Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.

Cancelling or changing an appointment

To cancel your appointment:

If you need help when we are closed

If you need medical help now, use NHS 111 online or call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

If you need help with your appointment

Please tell us:

  • if there’s a specific doctor, nurse or other health professional you would prefer to respond
  • if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • if you need an interpreter
  • if you have any other access or communication needs

Evening service

Appointments are also available two evenings a week between 6:30pm and 8pm.

It is not an emergency service. For emergencies, you will be referred to the out of hours service.

Please note, the dispensary is not open during evening surgeries and there is no chaperone available.

Home visits

If you wish to request a home visit, please use the Klinik service.

Please try to submit your request before 10am if you think you might need a home visit.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed.

You can be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
